Extended Medical & dental Benefits

| At the end of the month of
your 65th birthday, your City of Hamilton extended medical and
dental benefits will terminate. At that time, you will receive Ontario Drug Benefits from the Ontario Government. If extended medical coverage and or dental coverage are preferred, you will be required to subscribe to one of the subscription services listed at The Group - City Benefits Expiration Letter |

The following forms are for OMERS retirees that have not attained the age of 65. |

Dental Claim Form Claimants requiring reimbursement for dental care expenditures must have this form completed by their dentist, or dental specialist. Original receipts and applicable supporting documentation must accompany all claims submissions. The completed form and receipts may be forwarded directly to Manulife Financial. |

Prescription Drugs, Prosthetic Appliances
& Medical Equipment:
Extended Health Benefits Claim Form Claimants seeking reimbursement for extended health care benefits such as prescription drugs can complete this form and submit it by mail or in person to the address listed on the form. Original receipts and applicable supporting documentation must accompany all claims submissions. The completed form and receipts may be forwarded directly to Manulife Financial. |
