Your health and dental benefits will continue until <insert last day of the month of your 65th birthday>. When your City sponsored health and dental benefits expire you will be allowed to convert your health and dental coverage to an individual policy without having to provide evidence of good health or undergo a medical exam provided you convert within the time period applicable to the plan. Several plans are available to choose from, namely Manulife, Municipal Retirees Organization of Ontario and PlanDirect. Manulife offers benefits through their conversion program called "Follow Me". For more information please call their office at 1-877-268-3763. This is guaranteed coverage, regardless of health, as long as you enroll within 60 days of the date that your City-sponsored coverage expires. You also have the choice of converting to health and dental benefits provided through the Municipal Retirees Organization of Ontario. To inquire about this option please call their office at 1-800-363-7861. To be eligible for the conversion privilege you must enroll within 90 days of the date that your City sponsored coverage expires. PlanDirect offers a series of health and dental plans. For more information please contact their office at 1-800-565-4066. To be eligible for Plan Direct coverage you need to enroll within 60 days of the date that your City-sponsored coverage expires. Please note, however, the above conversion plans are not connected in any way with the City of Hamilton and the benefits coverage they offer is unrelated to any benefits the City offers to its retirees.
